CUSTOM ORDER FORM Patricia’s will provide you with a student order form to make copies as needed. The student order form will be
complete with the picture of your field day design and specific ordering instructions that you provide. Just let us
know your field day date, payment method(s) accepted, final day you will accept t-shirt orders, amount you
will be charging for shirts and any other special instructions you may have. Print out, fill in the highlighted areas and fax it to us, E-mail the necessary information or Call customer service with the information.
