CUSTOM ORDER FORM

Patricia’s will provide you with a student order form to make copies as needed. The student order form will be complete with the picture of your P.E. design(s) and any specific ordering instructions that you provide. Just let us know your order due date, payment method(s) accepted, amount you will be charging for shirts and any other special instructions you may have.
